Entries: Title, author's name (and co-author's, editor's, or translator's names), year published, publisher name and location, name of series (if applicable), story type (subgenre), subject, geographical and time settings, list of all major characters with descriptors for each, plot summary (about 200-300 words), citations to journals and magazines that have reviewed the title, list of up to five other titles by the author, list of up to five similar titles by different authors.
